Consensus Guidelines for Inpatient Management of EB

Dermatology experts from Cincinnati Children’s joined a group of more than 30 leaders in the field who have produced a unified set of recommendations for the inpatient management of epidermolysis bullosa (EB) in neonates and post-neonates.

The Cincinnati Children’s representatives included Anne Lucky, MD, Beth Moeves, APRN-CNP, Nichole Halliburton, APRN-CNP, and Emily Gorell, DO, MS.

Using a modified Delphi method, experts from the EB Clinical Research Consortium centers reached consensus on 103 out of 119 statements for neonates and 105 out of 122 for postneonates. The guidelines aim to enhance the quality of inpatient care for EB patients, emphasizing the importance of tailoring recommendations to individual clinical contexts.

“Hospital team members benefit from consensus-derived guidelines to care for patients with fragile skin,” Lucky says. “Our study provides the first expert-based consensus guidelines for neonatal and postneonatal care of EB in the inpatient setting in North America.”

The co-authors agreed that procedure risk, EB subtype, severity, and patient/family preferences should be considered when incorporating the guidelines into clinical practice, and that medical emergency management takes priority and may require deviating from guidelines. Procedural care for patients with EB requires advanced notice to operating room teams for proper preparation, including addressing advanced directives before interventions.

Future directions include complementing these guidelines with visual aids and demonstrations, analyzing improved patient outcomes after guideline incorporation, and developing consensus statements for outpatient care of EB.

Co-authors from 14 institutions across the U.S. and Canada also were involved in producing these guidelines.
